Legally, you need a medical degree, valid registration with the medical council, professional indemnity insurance and an authorised practice where you can practise. In practice, you need training in specific techniques, the judgement to select patients, a protocol for managing complications and, above all, patients. This guide sets out these elements in the order in which they usually come into play.

What the law requires and what it does not

In Spain, aesthetic medicine is not a specialism covered by the MIR programme. Any registered doctor may practise it, although the autonomous communities and medical associations recognise it as a medical activity with specific requirements regarding the practice and advertising. There are master’s degrees and university-specific qualifications that help to structure training and which some centres and insurers value, but they are not a legal requirement for practising. What is essential, however, is that the centre where you work is authorised to provide aesthetic medicine and that your insurance explicitly covers the treatments you carry out.

Phase 1: basic training (three to six months)

Applied facial anatomy: facial muscles, fat compartments, blood supply and risk areas.

Botulinum toxin in the top third: indications, dosage, injection sites and complications.

Hyaluronic acid fillers: rheology, planes, cannula and protocol for vascular occlusion using hyaluronidase.

Standardised clinical photography and informed consent.

Peel bases, mesotherapy and biostimulants as a second layer.

Choose courses that include supervised practical training with real patients and are taught by practising doctors. Avoid collecting certificates without putting what you’ve learnt into practice between one course and the next.

Phase 2: first patients with a safety net

It’s best to carry out your first treatments under the guidance of someone you can contact. This could be a supervising doctor, a masterclass with follow-up support, or a group of experienced colleagues. Document each case with photographs, noting the product, batch number, dose and treatment plan; this is your best learning tool and your best defence should anything go wrong.

Make sure you have the protocol for managing complications ready before seeing your first patient: hyaluronidase in the surgery, a written protocol, and the contact details of an ophthalmologist and the relevant A&E department.

Stage 3: where to practise

There are three approaches, and they are not mutually exclusive:

Work as an employee or on a contract basis at an already authorised clinic. You’ll learn about client volume, scheduling and management without having to invest anything.

Rent a surgery by the hour, day or month at an authorised centre. You provide the product, insurance and patients.

Open your own centre, with the health authorisation the relevant one. It is the option offering the most control and requiring the greatest investment.

Most people start with the first or second stage and move on to the third once they’ve consistently completed two or three sessions a week.

Stage 4: insurance, materials and suppliers

Sign up for or upgrade your third-party liability insurance so that it covers the specific treatments you’re going to carry out. Only buy products from authorised distributors, complete with delivery note, batch number and traceability. And it organises the recording of every step from the outset: patient, product, batch, dose, area, photographs and consent.

Stage 5: patients and advertising

Your first patients usually come to you on the recommendation of other patients, fellow doctors and people you know. Whenever you want to advertise your practice, check the following first: health advertising regulations: the centre’s registration number must be clearly displayed; no promises of results; no offers encouraging the use of medicines; and particular care must be taken on social media.

Common mistakes when starting out

Learn ten techniques before mastering two.

Treat high-risk areas (nose, glabella, temporal region) without first building volume in safe areas.

Working in a centre whose licence does not cover cosmetic medicine.

Not having hyaluronidase available in the surgery.

Advertising the results or prices of medicines on social media.

Failure to document cases.

How much does it cost to get started?

It depends on the route you take. As a practitioner working in a clinic, the costs are training and insurance. If you rent a practice, you’ll also need to factor in the rent and the initial stock of products. If you run your own centre, you’ll need to consider premises, building work, licences, equipment and months without income; we go into more detail in How much does it cost to set up a cosmetic medicine practice?.

Learning from other doctors who have already been through this

Medars is a private platform for registered doctors: masterclasses delivered by practising doctors, a forum where you can discuss cases with colleagues, verified suppliers and consultation rooms available for hire. Basic membership is free for life and is activated once your registration has been verified – no card required. Request access.

Related guides

Botulinum toxin training for doctors: what a reputable course should include

Requirements for setting up a cosmetic medicine practice in Spain

A Master’s degree in aesthetic medicine or a masterclass: which course to choose and when

How much does it cost to open an aesthetic medicine clinic?

Working as a self-employed cosmetic doctor in various clinics

Continue exploring Medars

Training for doctors: masterclasses on botulinum toxin, hyaluronic acid fillers and facial contouring, delivered by practising doctors.

Memberships: Basic is free forever, and Gold and Black are available if you want more access.

The Lounge: clinical cases with colleagues, direct messages and clinic rooms to rent.

The Shop: verified laboratories and distributors.

Teach at Medars: publish your masterclass and keep 70% from each sale.

Frequently asked questions: verification, access and memberships.

A guide for doctors wishing to start a career in aesthetic medicine: legal requirements, phased training, first patients, where to practise, insurance and advertising.

What you need to practise aesthetic medicine as a registered doctor: phased training, your first patients, where to practise, insurance, equipment and common mistakes.
[3

post-782","post","type-post","status-publish","format-standard","hentry","category-guias
["GET
