MEDARS CRM

Digital informed consent for your cosmetic medicine clinic.

The patient reads it and signs it with their finger on the tablet, in their own language. Your signature is already there, the text is sealed, and the document is saved in their file.

HOW IT WORKS

From form to signature in a minute.

1. You prepare it

From the patient’s record or from the appointment. The logo and details of the clinic, the patient and the healthcare professional are filled in automatically, as are the product, brand and batch number if they are taken from the inventory.

2. Open it on your tablet

A single-use link, valid for two hours, opens a full-screen page. The patient does not need to log in or install anything.

3. The patient signs

Choose your language, read the document, tick the boxes and sign with your finger.

4. It is sealed

The text, signatures, date, time and SHA-256 hash are saved. A signed consent form can only be revoked; it can never be edited.

MODELS INCLUDED

The consent forms used by a cosmetic medicine clinic, already drawn up.

MEDARS CRM includes the templates used by a real clinic, translated into eight languages: Spanish, English, French, German, Italian, Portuguese, Chinese and Russian. Please review them with your consultant before using them and adapt them to suit your practice.

Injectable treatments

Nose reshaping, polylactic acid, calcium hydroxyapatite, hyaluronidase, hyaluronic acid and botulinum toxin, also for bruxism and hyperhidrosis.

Other procedures

Chemical peel and local anaesthetic.

General

General consent, use of photographs and data protection.

DOUBLE SIGNATURE

Your signature, provided once only.

Sign each model once

Each professional signs their forms on screen or uploads an image of their signature. That signature is then automatically included on every consent form prepared for them.

If the text is changed, it is re-signed

When you edit a template, the previous signature no longer applies to the new text.

Without both signatures, it’s invalid

One consent form, which only requires the patient’s signature, is still pending and will be finalised as soon as the healthcare professional signs it.

WITH THE MEDICAL RECORD

Consent, wherever you need it.

A note before we begin

When you start the appointment on the tablet, the CRM alerts you if the signed consent form for that appointment is missing.

Photos for social media, with permission only

A clinical photograph cannot be flagged for marketing use without a signed image consent form. This is verified by the database.

Everything in the fact sheet

Signed consent forms appear in the patient’s record and in the exported version of their medical records.

QUESTIONS

Frequently asked questions.

Does the patient need to install anything or create an account?

No. You open the consent form on the clinic’s tablet using a single-use link, valid for two hours, and the patient signs on the full-screen display without logging in.

Can I use my own consent forms?

Yes. You can edit the included templates or create your own. The fields for the clinic, patient, practitioner and product are filled in automatically.

What is recorded when the patient signs?

The exact text you read, your signature, the professional’s signature, the date and time, and an SHA-256 hash of the document. None of this can be edited afterwards.

What if the patient changes their mind?

Consent is revoked with a date. The signed document is not deleted: it remains in the record, marked as revoked.

In which languages are the templates available?

Spanish, English, French, German, Italian, Portuguese, Chinese and Russian. The patient chooses the language before reading.
